Survey field with "Other" option and text field.

We are looking for a field that does the following:

We are trying to have an end user defined field value in a radio button so we can allow either a statically defined choice, or a choice the user can write in their defined option. Is this in any of your plugin packs? If not do you know if this is on the market, or if you are investigating anything like this?
